What is a MnCHOICES Assessment?

A MnCHOICES assessment is a free evaluation completed by a nurse or social worker to determine the level of care a person needs.

During the assessment, the evaluator may ask about:

  • Mobility and physical health
  • Memory and cognitive concerns
  • Daily activities such as dressing, bathing, and eating
  • Medication management
  • Social and emotional needs
  • Current support systems

This assessment helps determine whether someone qualifies for Elderly Waiver services and what types of support may be appropriate.

EW may help pay for supportive care services such as:

  • Assistance with bathing and dressing
  • Medication management
  • Housekeeping and laundry
  • Meals and nutrition support
  • Transportation
